Primary Card Applicants

1. A photocopy of I.D.
   
2. A photocopy of household registration (First page and the page where the applicant’s name appears)
   
3. A copy of the latest computerized payroll slip or salary letter certified by the employer or bank statement showing salary payment for the previous 6 months

Supplementary Card Applicants : A photocopy of I.D.

1. Bank statement showing at least 700,000 Baht monthly turnover during the previous 6 months
2. The applicant’s current or savings account must be opened and maintained for more than 1 year
3. Home or office telephone number must be provided

1. A photocopy of I.D.
2. A photocopy of household registration (First page and the page where the applicant’s name appears)
3. A copy of enterprise registration or trade registration, showing the applicant’s name
4. A copy of bank statement for the previous 6 months and the page indicating the account's name and number. In case of company’s bank statement, please submit shareholder list.
