1. Age between 20-65 years old
2. Hold medical license
3. Doctors who work for private hospitals or public hospitals must have minimum monthly income 15,000 Baht. Doctors who are self-employed must have bank statement showing at least 50,000 Baht monthly turnover.

Requisite Documents

1. One photo sized 2  inches, taking from the front side
2. A photocopy of I.D.
3. A photocopy of the applicant’s medical license 
4. A copy of latest computerized payroll slip or salary letter certified by the employer. For self-employed, a copy of bank statement for the previous 6 months and the page indicating the account’s name and number.

Remark : Krungsri GE Visa Doctor Card is for Doctor who hold medical license) only. Thus, there will be no supplementary card.
